Spring Sports Paperwork

All spring sports paperwork is due in the nurse’s office no later than February 15th. Paperwork can be found on the school’s website under the “nurse’s office” tab. You MUST submit paperwork for every new season of play. Sports Physicals are good for one complete year. Please check with the nurse to see if you have a good physical on file. If you have any questions or concerns about physicals or paperwork, please see the nurse BEFORE February 15th.
