Fall 2011 Sports Physical Packet & Instructions

Please read the following important information.

Parents & students must complete the Activity/Athletic Eligibility Form, the Health History Questionnaire (Part A), and the Steroid Testing Form. These three forms must be completed for each sport and season.  Sports physical packets are due by August 1, 2011. If you had a physical after August 16, 2010 and it is already on file in the Marlboro High School Health Office, you do not need to have a new physical. Check with the nurse immediately if you are not sure; however, the rest of the packet is still due.


You must have a physical filled out by your doctor on our physical evaluation form -Part B.  Sports physicals will be accepted only on the NJ State Form (Part B). This form must be completely filled out by your doctor, including height, weight, blood pressure, pulse, and vision. If you wear glasses or contact lenses, bring them to your exam along with the completed Part A form which MUST be reviewed by your physician. A signature and stamp from your physician’s office is required. If your child is cleared, box A should be checked by your doctor. It is your responsibility to see that the sports packets are completed in their entirety.  Incomplete physical forms will be rejected. 

Pursuant to state law N.J.A.C. 6A:16-2:2(h), in order to have a physical performed by the school doctor, parents must sign a Waiver which states that you have no medical home (defined as a healthcare provider). You may obtain the Waiver form from our health office, have it signed, and returned by July 25, 2011.

Please note:  all forms are to be returned to the health office.  If you are handing in your sports packet during the summer, please make sure that it is brought to the main office.

Main Office Hours are 8:30 a.m. to 3:00 p.m.
